Alvin Cummins of Waddy, Kentucky was indicted by a Shelby County grand jury on one Class D felony count of filing a false Kentucky individual income tax return for the 2017 tax year. Mr. Cummins was previously indicted by a Shelby County grand jury on February 5, 2018, on four Class D felony counts of filing a false Kentucky individual income tax return, one count each for the years 2013 to 2016. He is currently awaiting trial on these previous charges. David Whitehouse conducted both of these investigations and presented them for prosecution.

Lesley A. Wade of Frankfort, Kentucky was indicted by a Franklin County grand jury on nine Class D felony counts of Willfully Filing/Making a False Tax Return or Failing to Pay Tax for tax years 2010 through 2018. The total amount of tax due for these years is $81,743. Including interest and penalties, the total tax liability comes to $140,791.39. David Whitehouse presented this case for prosecution.

Cheryl Justice of Louisville, Kentucky was indicted by a Franklin County grand jury on three felonies of Theft by Failure to Make Required Dispositions of Property. Two of the charges were Class D felonies and one was a Class C felony. These charges are related to sales tax from a business, Gypsy’s Sports Grille, of Louisville, Kentucky that were collected and never remitted to the Commonwealth of Kentucky. The total amount of theft was $29,705.64. With interest, penalties, and fees, the total liability comes to $60,726.73. Alex Peyton presented this case for prosecution.
