The Kentucky Department of Revenu​e is focused on protecting you and your identity.  Kentucky has implemented many internal measures along with partnering with outside sources to prevent tax fraud and identity theft.  Fraud and identity theft cases are growing each year but Kentucky is analyzing and updating procedures as needed.  Safeguarding your tax information is very important to the Department of Revenue.

Identity Confirmation Quiz

Kentucky has implemented an identity confirmation quiz.  If you are selected to take an identity quiz you will receive an identity verification letter asking you to complete the quiz within 30 days.  The quiz consists of multiple choice questions with answers that help us verify your identity.  The quiz can be taken online or by telephone with an authorized representative to help with the process.

At the completion of the quiz you will be advised whether you passed or failed.  If you passed, processing of the refund continues.  If you fail the quiz on your first attempt, a second quiz using different questions will be available..

If you do not pass in two attempts or during the 30-day period you will be required to provide identity verification in the form of a government-issued ID such as a driver's license or passport and proof of withholding in the form of employer-issued W-2s, 1099s, etc. to DOR in order to complete the processing of the refund. 

The requested refund will not be paid until the quiz is passed or we receive proper documentation.  

If you received the identity confirmation quiz letter and have not yet filed a current year Kentucky individual income tax return, check the appropriate box in the letter and return the letter to the Department of Revenue.  You may also report this on the identity confirmation quiz website.
