How to File Your Return

Victims of identity theft must file a paper individual income tax return (forms may be found here) and include the following:

  1. a completed IRS Form 14039 (Identity Theft Affidavit)
  2. schedules that apply to your return (Schedule A, M, P, etc.)
  3. withholding statement(s) such as Schedule KW-2 along with W2s, W2g, 1099s, PTE-WH, etc.
Mail all documents to the address on the return.
